Human Performance as a Root Cause
Understanding what errors or defects occur is a must to be able to fix those issues. It is the essence of continuous improvement; you want a culture that celebrates errors found to be able to improve them.
Without this culture that celebrates process defect discovery, you will create a factory of fear!
This limits progress and determines that process defects are due to human failure, in Lean Six Sigma we live by the phrase “Process, not the people” This helps invite the right culture.
If you think about the problem and what they are not being accountable to you will see it is the process that allows colleagues to not be accountable for their actions and we have to make that process more efficient, so it does not allow colleagues to make the same mistake.
Create a process that doesn’t allow more human error than below:
Complex non-routine task (1 error in 10 = 10%)
Routine task with medium complexity (1 error in 100 = 1%)
Routine simple task (1 error in 1000 = 0.1%)
Simplest possible task (1 error in 10,000 = 0.01%)
Accountability and Communication are symptoms of a root cause!
